From the US to another US state, usually a week or two for custom stuff. I got a ring last month and it was pretty quick, like 8 days total.

I think for the most part it's handmade. I've bought a few things and they always felt unique. If something looks too perfect or like it came from a factory, I'd probably avoid it or ask the seller.
